To get started, follow these steps.

 

Step 1: Schedule an appointment.  When you do so, we will send you a password to enable you to download and print the New Health Partner Packet.

 

Step 2:  Please print out the Forms in the New Client Packet and fill them out.

 

Step 3: Then deliver, email or mail them to me at least 3 days before your first appointment.
